The primary reason I bought the X4 Labs extender was the hybrid support piece, you have the option of trying the noose or the strap or both together. I did alot of research before buying any extender, I read most of the manufacturer's "user forums" and found that there was a split choice between the strap and the noose. The decisions these users made were based on their penis structure, comfort and their percieved effectiveness; as I didn't have anything in hand to sample, I felt the X4 gave me the most choice. I started with the strap, then started using the noose; eventually all were worn and unusable. When I was looking into buying replacements, I decided on the quad support piece which included two straps, two nooses and the option to mix and match. I'm still using the quad support piece only because of the longer bed which I find more comfortable and a strap and noose together at the "regular position". The reason I use both together is that I find the task of holding the penis in place is distributed between the noose and strap, it extends the life of the strap and the noose and even the foam for the noose because you don't have to strap in as tightly. I'm still using the same noose, noose foam and strap since September 16, 2010; just the fact that the foam has held together for four months at 1600g is amazing.
