I am a recent customer, am actually awaiting the shipment to arrive. I have elected to participate in your clinical trial offer during my use of the product. I have received and printed off the forms that were emailed me regarding the clinical trial - thanks for the quack response and convenient distribution of those forms. The one thing I noticed is that the forms do not list ALL of the requirements to participate in the clinical trial. On your website the offer details page spells out additional requirements that might disqualify a lot of participants if they don't search online, find these and only send in the forms and info included in your email. It would be appreciated if a complete terms and conditions page (similar to what is listed online) could be included in the automated clinical trial email - as well as any other stipulations needed to fully comply with your offer.

I'm concerned about this as well. Was there any difficulty or delay in getting your refund?
