It is uncomfortable to tighten/loosen the base screws while wearing the thing.
But I think that using a hydroulic (or air-based) type of system instead of the screws would be better. It could use something like the old nike shoe pumps to pump air into the inside chamber that pushes against the other rods (extending the extender). Only thing I can think of where it might not be such a hot idea is that hydroulics are usually used for crushing cars and air pumps gradually lose air.
